ما هو الـ آي ان آر (INR)؟

دايما هنسمع عنه لما يجي دكتور يركب CVP وبالذات لمرضي الكبد والقلب

ايه هو بقي؟
هى اختصارلـ:International normalized ratio
The INR is the ratio of a patient's prothrombin time to a normal (control) sample,
raised to the power of the ISI value for the control sample used.
(evaluate the ability of blood to clot properly)
ويسمى مقياس مدى السيولة للدم
(International Normalized Ratio).
وبدل الكلام الكتير ده نقول انه
rate of blood tendency to form clots specially if patient takes warfarin

ما هى اهمية الـISI؟؟؟؟
International Sensitivity Index
أوصت منظمة الصحة العالمية بهذا الأسلوب فى حساب الINR
بعد أن وجدت اختلافات كبيرة في أزمنة البروثرومبين لنفس المريض باختلاف الشركات الصـــانعة لكواشف الثرومبوبلاستين
يعني لو الواحد جاب نتيجة تحاليل الPT او ال PTT مختلفه عن INR يبقي السبب هو ISI اللي هي الماده اللي بتاع المعمل بيضيفها

A high INR level such as INR=5 indicates that there is a high chance of bleeding,
whereas if the INR=0.5 then there is a high chance of having a clot.
Normal ranges for a healthy person are 2.0-3.0 and for people on warfarin therapy is 2.5-3.5
(although the target INR may be higher in particular situations, such as mechanical heart valves).

ولهذا الاختبار مدى هو 2 إلى 3 في أكثر الحالات و 2.5 إلى 3.5 لحالات عمليات الصمام الصناعي القلبي . وبتأثر فعالية الدواء يتأثر هذا الاختبار.
كلما قلت قيمة الاختبار قلت السيولة و العكس صحيح.
ما هي أهمية مقياس مدى السيولة (INR),ومتى يُنصح المريض بعمل الاختبار؟
أهميته
تكمن في إعطاء مقدمي الرعاية الصحية صورة واضحة عن سيولة الدم للمريض و بعد ذلك حساب الجرعة المثالية للمريض.
يجب عمل الاختبار كل يوم من أول جرعة ولعدة أيام
بعد ذلك مرة كل أسبوع حتى ثبات سيولة الدم إلى المستوى المطلوب.
بعد ذلك يعمل الاختبار كل فترة قصيرة.

ما هي الأشياء التي تؤثر على مقياس مدى السيولة (INR)؟
هناك عدة أشياء تؤثر على الاختبار منها المرض و الأكل و أدوية أخرى و حالة النشاط الجسماني .
يجب إخبار مقدمي الرعاية الصحية عن أي تغيير في نمط الحياة و الحالة المرضية و الأدوية الأخرى.

International Normalized Ratio Blood Test-INR

Normal INR Values: 1 to 2
The INR is used to make sure the results from a PT test is the same at one lab as it is at another lab. In the 1980’s the World Health Organization determined that patients may be at risk because the results of a PT test would vary from one lab to another, based upon the way the test was done. The “normal” range for one lab would be different than a “normal” value from another lab, creating problems for patients who were being treated in several locations. In order to standardize the results between labs, the INR was created. The INR result should be the same, regardless of the location where the tests are performed

ايه هو بقي الوارفارين؟(Warfarin):
الوارفارين (Warfarin) هو مادة مضادة لتخثر الدم. و دواءٌ يستخدم لزيادة سيولة الدم وذلك عن طريق منع تكون الجلطات الدموية (تخثر الدم) أو منع نموهــا.
فالوارفارين يعتبر دواء مضاد لتأثير فيتامين Kوهو عنصرضروري جداً في تصنيع عوامل التخثر رقم 2 ورقم 7 ورقم 9 و عامل رقم 10
الوارفارين (Warfarin) يصرف في أكثر الأحيان للمرضى المصابين بعدم انتظام ضربات القلب أو بعد الإصابة بالنوبة القلبية أو بعد العمليات الجراحية للقلب

Prothrombin Time Blood Test-PT

This test is done to evaluate the blood for its ability to clot. It is often done before surgery to evaluate how likely the patient is to have a bleeding or clotting problem during or after surgery.
Normal PT Values: 10-12 seconds (this can vary slightly from lab to lab)
Common causes of a prolonged PT include vitamin K deficiency, hormones drugs including hormone replacements and oral contraceptives, disseminated intravascular coagulation (a serious clotting problem that requires immediate intervention), liver disease, and the use of the anti-coagulant drug warfarin. Additionally, the PT result can be altered by a diet high in vitamin K, liver, green tea, dark green vegetables and soybeans.

Partial Thromboplastin Time Blood Test-PTT

This test is performed primarily to determine if heparin (blood thinning) therapy is effective. It can also be used to detect the presence of a clotting disorder. It does not show the effects of drugs called “low molecular weight heparin” or most commonly by the brand name Lovenox.
Normal PTT Values: 30 to 45 seconds (this can value slightly from lab to lab)
Extended PTT times can be a result of anticoagulation therapy, liver problems, lupus and other diseases that result in poor clotting.
